A Retinal Detachment Case with a Delay in Diagnosis due to Myelinated Nerves

It is aimed to report a case of retinal detachment with delayed diagnosis due to myelinated nerve fibers. A 35 years old patient with complaints of blurred vision was referred to our clinic for visual field testing with a diagnosis of possible glaucoma and myelinated nerves. Visual acuity was full in both eyes, biomicroscopy was nonrevealing and intraocular pressure was 14 mmHg OD and 15 mmHg OS. The patient had visual field defects in the inferior and superior nasal field and superior temporal field periphery. There was myelinated nerve fibers observed in retina examination around right optic nerve. Right eye was reexamined as the myelinated nerves was not expected to cause visual field defects correlated with the patient’s clinical picture. There was a shallow detachment in the superotemporal periphery and a break at 11 oclock position was found. Pneumatic retinopexy was performed. Retinal detachment was attached. The visual field defects disappeared on follow-up. The importance of careful retinal examination in cases with visual field defects uncorrelated with clinical picture should be kept in mind.
