The Role of Pars Plana Vitrectomy After Unsuccesful Complicated Detachment Surgery

Purpose: To evaluate the results of pars plana vitrectomy in patients with complicated retinal detachment after conventional scleral buckling surgery.

Materials and Methods: A hundred and four eyes of 103 patients (73 male and 30 female) who underwent pars plana vitrectomy between January 1996 and December 2004 and a minimum follow-up of 6 months were included in this study. The surgical technique used in operations, anatomic and functional results, and complications were retrospectively analysed.

Results: Sixty one (58.7%) cases were phacic, 31(29.8%) were pseudophacic and 12 (11.5%) were aphacic. Eighty seven had circumferential scleral buckling and seventeen had only segmental buckling. The avarage number of vitreoretinal procedure was 1.32 (1-3). The retina was attached in 75 (72.1%) eyes in the last follow-up visit, macula was attached with a peripheral localised detachment in 21 (20.2%) eyes and the retina was detached involving macula in 8 (7.7%) eyes. Visual acuity in the final visit was 0.1 or better in 35 (33.7%) eyes, counting fingers in 49 (47.1%) eyes and the level of hand motions or worse in 20 (19.2%) eyes.

Conclusion: Results of vitreoretinal surgery in cases with complicated retinal detachment after scleral buckling surgery was satisfactory with the use of perfluorocarbon liquids, gas or silicone oil tamponade in terms of functional and anatomical success.
