Posterior microphthalmos is an autosomal recessive inherited eye disorder which is a rare form of microphthalmos. In posterior microphthalmos, posterior segment development is aff ected while anterior segment development is normal. The main findings are high hyperopia and papillomacular folds. In this study, clinical and imaging findings of three sisters with posterior microphthalmos were evaluated. High hyperopia and total short axial length revealed in all three cases. Anterior segment examinations were normal. Papillomacular folds revealed in two of three cases. Optical coherence tomography demonstrated the papillomacular folds. Crowded optic disc were noted in all cases and retinal pigment epithelium changes were noted in two cases. This situation may be overlooked because of the normal anterior segment appearance. So it should be considered in patients presenting with high hyperopia. Patients with posterior microphthalmos should be closely monitored for potential complications. Keywords : Microphthalmos
