An 8-year-old boy was admitted to our clinic with a complaint of low vision. Anterior segment and gonioscopic examinations of the patient who had high hyperopia were normal. Shortening of the total axial length and diffuse thickening of the choroid in both eyes were detected with A and B mode ultrasonography. Bilateral crowded optic disc appearence, papillomacular retinal folds and increased retinal vascular tortuosity were observed on dilated fundus examination. Corneal topographic evaluation revealed thickened central corneas and slightly increased keratometric values. Measurements of the angle opening distance and trabeculo-iris surface area with anterior segment optic coherence tomography (OCT) were normal. Increased peripapillary nerve fiber layer thickness was noted with optic nerve head OCT analysis. There were marked neural retinal folds on macula OCT and these showed hypoautofluorescence on fundus autofluorescence imaging. Keywords : Posterior microphthalmos, papillomacular retinal folds, anterior segment optical coherence tomography
