Nonarteritic Anterior Ischemic Optic Neuropathy in a Young Patient with Dysfibrinogenemia and Homocysteinemia

A 47-year-old female patient was admitted to our clinic with the chief complaint of sudden onset visual loss in the right eye that had occurred 25 years ago. Her visual acuity was count fingers OD and was 1.0 OS with the Snellen chart. The anterior segment evaluation showed normal OU. There was optic atrophy in the OD and the fundus examination showed a normal OS. There was a relative afferent pupillary defect (RAPD) in the OD. The patient had had two still-births and two normal deliveries in her past medical history. A laboratory work-up was performed with a preliminary diagnosis of nonarteritic ischemic optic neuropathy and mutations were found in the methylene tetrahydrofolate reductase (MTHFR) C677T and β- fibrinogen genes. Younger patients with NAION should be examined for thromboembolic diseases and preventive measures should be taken if any diseases are found.
