Conservative Treatment or Surgery in Delayed Suprachoroidal Hemorrhage?

Suprachoroidal hemorrhages are one of the most devastating complications of intraocular surgeries. Here we present a patient who developed “delayed suprachoroidal hemorrhage” after an uncomplicated scleral fixation intraocular lens implantation surgery and we discuss treatment options in this type of hemorrhages.
An aphakic 65 years old male patient developed a sudden loss of vision 3 days after the secondary IOL implantation surgery. In B mode ultrasonography of the eye, massive hemorrhagic choroidal detachment was detected. Resorption of the hemorrhage started 10 days after topical and systemic corticosteroid and topical cycloplegin drops.
Prognosis in delayed suprachoroidal hemorrhages usually depends on the intensity, localization and the duration of the hemorrhage. In this case because the patient responded well to the conservative treatment after the tenth day, we did not plan any surgical approaches. Following with conservative therapy will decrease any further surgical treatments in particular group of patients.
